Who this is for

You have thought about the medical care you would want if you could not speak for yourself, and who you want speaking for you. Now you want it written down properly and signed.

Sometimes the timing is not your choice. A parent is going into surgery, or a hospital social worker has asked the family whether a directive exists. Deanna can prepare the document the patient directs, and the signing can happen at the bedside.

What the service covers

Deanna Wilson, a registered and bonded Legal Document Assistant, prepares the directive using the names, choices and instructions the signer gives her. The decisions in it belong to the signer alone.

Tell us how the directive is meant to be signed, whether with a notary, with witnesses, or both, so we can plan the appointment. When notarization is part of it, Marcie Ramirez comes to the signer.

  • Preparation of the directive at the signer's direction
  • A check of names, spellings and pages before printing
  • A signing appointment at home, in the hospital or at a care facility
  • Notarization when the document calls for it, with each additional notarized signature at $15

What to have ready

  • The signer's own decisions about their care and who will speak for them. Deanna cannot make or suggest these choices.
  • Full legal names and contact details for anyone the signer names.
  • A current government issued photo ID for the signer, such as a driver license, state ID card or passport.
  • The name on the document should match the name on the signer's ID.
  • The signer must be alert, understand the document, and want to sign it.
  • Do not sign before the appointment.
  • If the document calls for witnesses, arrange for them to be there and ask whether any rules limit who can serve.

How it works

  1. 1

Tell Deanna what the signer wants prepared

Call (619) 345-0006 or book online. Tell us where the signer is and how soon the document is needed.

  2. 2

    Deanna prepares the directive

She prepares it at the signer's direction and checks every page.

  3. 3

    The signer reviews it

The signer reads the document or has it read to them. Anything that does not match their instructions is corrected.

  4. 4

    Signing where the signer is

At home, in the hospital, or at a care facility, planned around how the document is meant to be signed.

Questions

Directive preparation with Doc Prep & Notary Services starts at $150, and each additional notarized signature is $15. We confirm your price before any work starts.

No. Those are personal and medical decisions, and choosing what the document says is outside what a Legal Document Assistant can do. Your doctor, your family and an attorney may help you think it through. Deanna prepares what you decide.

Yes, as long as the patient is alert, understands the document, and is willing to sign. Let the nurse know a signing is planned so it can be scheduled around care.

How a document must be signed is part of its legal effect, and we cannot advise on that. Tell us what your document calls for, and we will plan the appointment to match. If you are unsure, ask an attorney.

They are separate documents, and we price them separately. Which documents fit your situation is a legal question we cannot answer. Deanna can prepare either one at your direction.
